Do you want to make a WordPress survey? Surveys allow you to collect valuable information that you can then use to make data-driven decisions. In this article, we will demonstrate how to easily create a survey in WordPress and generate stunning reports.
Choosing the Best WordPress Survey Tool
You can create survey forms and embed them in WordPress using a variety of online services. The disadvantage of these forms is that the data is stored on third-party servers, and customization options are limited.
Some of these forms also have a limit on the number of responses that can be collected. You must pay extra to receive more responses, and these services are typically expensive on a monthly basis.
Wouldn’t it be great if you could run surveys on your own website instead of having to rely on third-party services?
WPForms can help with this.
They recently released a powerful new WordPress surveys and polls addon, which allows you to easily create survey forms on your website.
You get the following features with the WPForms survey feature:
- To create surveys, use this powerful drag-and-drop form builder.
- Rating, Likert Scale, and other intelligent survey fields
- Conditional logic is used to customize survey fields based on user input.
- Survey reports that are interactive and include bars, graphs, charts, and tables
- Survey graphs can be exported as JPEG, PDF, or print formats.
- Export survey results as a CSV file for later use.
That being said, let’s look at how to quickly create and add a survey to your WordPress website.
WPForms was created by the same team that created Your Blog Master. It’s one of our top-tier WordPress plugins.
Using WordPress to Create a Survey
The very first step is to install and activate the WPForms plugin. View our step-by-step guide on how to install a WordPress plugin for further information.
The WPForms surveys and polls addon is included in their Pro plan. Your Blog Master users can get a 10% discount on all WPForms licenses by using our WPForms coupon.
After activating the plugin, navigate to the WPForms » Settings page to enter your license keys. You can find his information on the WPForms website in your account.

After that, go to WPForms » Addons and scroll down to the Surveys and Polls addon. Install the survey addon by clicking the Install addon button. Once installed, you must activate the addon by clicking the ‘Activate’ button.

You are now ready to design a survey form.
To create a new form, go to the WPForms » Add New page. This will open the drag-and-drop form builder interface in WPForms.

First, enter a title for your survey form and then select the Survey Form template.
This will load a sample customer feedback form template with several fields pre-populated.

Simply point and click to edit any field, drag and drop to rearrange them, or remove them from the form. From the left column, you can also add new form fields.
WPForms supports all common form fields, such as drop down, radio buttons, checkboxes, likert scale, text input, rating scale, and many others. This makes it extremely simple for you to create highly interactive survey forms.
WPForms also supports conditional logic, allowing you to tailor your survey questions based on user input in one of the previous fields.

After you’ve finished creating the form, click the save button to save your form settings. You can now exit the form builder by clicking the close button.
Including a Survey Form on Your WordPress Site
WPForms makes it incredibly simple to add forms to any page on your website. Your survey form can be easily added to any WordPress post, page, custom post type, or sidebar.
To include a survey form in a WordPress post, page, or post type, go to the post edit screen and select the ‘Add Form’ button.

This will open a popup where you can select the survey form you created earlier and continue by clicking the insert form button.
The form shortcode has now been added to the post editor.
Once you’ve finished editing the post, click the publish button. You can view your survey form in action by going to your website.

WPForms also makes it simple to insert your survey form into any sidebar or widget ready area. Simply go to your WordPress admin area’s Appearance » Widgets page and drag the WPForms widget to any sidebar.

Select the survey form you created earlier in the widget settings, and don’t forget to click the save button to save your changes.
You can now go to your website and see your survey form in the sidebar.

Viewing the Results of Your Survey Form
WPForms displays survey results in visually appealing charts and graphs. You can also print survey results and export them to your preferred spreadsheet software.
To view your survey results, go to WPForms » All Forms and click the ‘Survey Results’ link below your survey form.

Your survey responses will be displayed in an interactive chart and tables on the results page. You have the option of exporting responses to a single question or the entire survey.

On the top, you’ll see options to switch between chart types as well as an export option. Individual survey results can be saved as JPEG, PDF, or Print files to be easily shared on presentation slides, blog posts, or social media.

Any form can be converted into a survey.
WPForms also lets you convert any form into a survey form and display previous results as a survey. You can even specify which form fields should be treated as survey fields.
This is especially useful if you want to collect data via other forms, such as a contact form, user registration, or newsletter sign up forms.
To make an existing WPForms form into a survey, simply edit it in WPForms and navigate to the Settings » Surveys and Polls section. Check the box next to the ‘Enable Survey Reporting’ option and save your changes.

To enable survey reporting for a specific form field, edit the form and then click on the field you want to enable.
On the left side, under the ‘Field Options’ section, click on the Advanced Options and check the ‘Enable Survey Reporting’ option.
